Chronic Oedema of the Lower Limb: Practical guidance on diagnosis, effective treatment and ongoing management
Compression therapy remains a cornerstone in the effective management of leg ulcers and chronic oedema, yet it is still underused or applied incorrectly in clinical practice. Evidence consistently shows that optimal, timely compression significantly improves healing outcomes, reduces recurrence, and enhances patient quality of life.
The EWMA initiative highlights the importance of improving awareness, education, and consistency in compression use. Key barriers—including delayed initiation, incorrect application, and lack of clinician confidence—continue to limit patient outcomes. Addressing these gaps through practical education and clear clinical guidance is essential.
By promoting best practice and supporting healthcare professionals with accessible resources, compression therapy can be more effectively integrated into routine care—ensuring better healing rates and improved patient outcomes across the continuum of care.

Understanding Etiology: What's driving your compression choices?
In the S.T.R.I.D.E compression framework, E represents Etiology and refers to the underlying cause or combination of causes driving a patient's oedema or lymphoedema. These may include lymphatic dysfunction, chronic venous insufficiency, immobility, obesity, or other systemic conditions. In clinical practice, mixed etiology presentations are common, meaning swelling is rarely due to a single factor.
Understanding etiology is fundamental. It shapes not only the presentation of swelling, such as distribution, tissue characteristics, and progression, but also how the patient will respond to compression therapy.
Implications for Compression Garment Selection
A clear understanding of etiology enables prescribers to move beyond one-dimensional focus on compression levels and instead tailors therapy more precisely by:
- Defining treatment goals
- Guiding fabric selection
- Supporting safe compression prescriptions
- Informing garment design and configuration
- Shaping long-term management

EWMA 2026: Global Perspectives on Compression and Oedema Management
The European Wound Management Association (EWMA) Conference 2026 brought together more than 6,200 delegates in Bremen, Germany, from 6–8 May, providing a valuable opportunity for clinicians, researchers, and industry leaders to share the latest advances in wound and oedema care. Australia was strongly represented, with a number of respected clinicians presenting their work and contributing to international discussions.
As part of the event, Stacey Bradshaw attended the Compression Therapy Programme International Expert Committee meeting, where she shared insights into compression practice in Australia and highlighted the success of the Beat the Swell campaign. Australian initiatives in oedema awareness and compression education attracted significant international interest, reinforcing Australia's leadership in this important area of care.
A standout feature of the conference was the creative awareness activities led by the Compression Therapy Programme. Committee members partnered with a professional choir to perform a compression-themed adaptation of Wouldn’t It Be Nice, engaging delegates and sparking conversations about the importance of compression therapy in clinical practice.
The conference program featured a strong focus on oedema and compression management, offering valuable insights into the varying approaches, challenges, and innovations being implemented across Europe. Beyond the scientific sessions, the event provided exceptional opportunities to connect with leading experts in wound care and gain a broader understanding of how wounds and chronic oedema are managed around the world.
